Company Policies and Procedures:
For your convenience we have listed our policies and procedures. Each will provide you with a detailed description of our responsibility to you as well as your responsibility as our client.
Please be advised that we do not massage any persons below the age of 18 and women who are less than 4 months into their pregnancy.
Appointments and Cancellations:
All appointments must be cancelled the day prior to your scheduled appointment. Please contact our office directly during normal business hours from 10 am to 9 pm to cancel or reschedule your appointment.
When you make your appointment please provide us with a valid phone number where you can be reached on the day of your appointment as we will call to verify your appointment time. If we cannot get in touch with you on the day of your appointment to verify that you will be in, you may lose your appointed time slot.
Although we understand that there are times when extenuating circumstances prohibit you from keeping a scheduled appointment, repeated missed appointments or cancellations without proper notice will prevent you from making any future appointments.
Appointment Day:
Please arrive at least 10 minutes prior to your scheduled appointment. Upon your arrival, please check in at the front desk.
Upon your visit you will be asked to complete the following forms. The city requires that we maintain these records.
Customer, Hot Stone Therapy, and/or Ashiatsu Oriental Bar Therapy Informed Consent Forms: The applicable forms must be completed and signed prior to any massage services being performed. Please fill these forms out accurately and completely as you may be asked for identification to confirm your name and address.
Daily Customer Log: Includes your name & address, date and time of service, practitioner’s name & treatment and customer consent.
Late Arrivals:
Because we schedule clients behind you a failure to keep scheduled appointments prohibits us from attending to and caring for your massage treatment needs properly during your allotted time. Late arrivals may be asked to reschedule their appointments for an alternate time or accept a lesser treatment time that will not interfere with the scheduled appointment behind you.
Payments:
Payment is required at the time services are rendered.
All payments are either made in cash, credit card, or gift card. WE DO NOT ACCEPT CHECKS, NO EXCEPTIONS!
Lost or Stolen Articles:
We are not responsible for lost or stolen articles or items.
Service Limitations:
We do not provide massage services to persons under the age of eighteen. We also do not massage women who are less than 4 months into their pregnancy. NO EXCEPTIONS.
Changes to Our Services:
We reserve the right to change our policies and procedures, our pricing, and our hours of operation at any time. We also have the right to refuse or discontinue service for any reason.
